Patient Access allows you to book appointments online, order prescriptions, view medical records and update your details.

To register for EMIS Patient Access you must download the form below and return the completed form in person to reception. 2 forms of ID must be shown with the form.

Please note this is only available for people over the age of 16. If Patient Access is not suitable for your circumstances we recommend speaking to your pharmacist to allow them to take over the routine ordering of your medication.
